Graduate of an accredited school of Vocation Nursing and/or passing LVN Board examination required. Minimum three years of experience in managed care, case and/or care management in an acute care hospital, IPA, Medical Group or Health Plan environment required. Home Health or Hospice experience preferred.

Knowledge of Medicare, Medi-Cal, DHS, MRMIB, DMHC and NCQA regulations and guidelines, MCG (Milliman), Interqual, Apollo Medical Guidelines, etc. Efficient use of computer, fax, telephone, copier, pager, paging and medical records systems. Must have excellent interpersonal skills professional demeanor and communication style consistent with essential duties of position. Requires critical thinking skills and must be able to be flexible to a variety of tasks and be well organized; must be able to prioritize work with strong decision making capabilities. Must be able to read and understand maps and directional instructions for hospital and home visits. Ability to communicate distinctly via telephone and writing, identify and distinguish various sounds, visual ability to read

Active, unrestricted Licensed Vocation Nurse (LVN) license required. Valid Driver’s License and automobile insurance required at time of hire. Complex Case Management (CCM) certification preferred.
